This is not the first time we are flying to Turkey with a child. But ... I don’t remember what the document was called earlier on permission for a child to travel without one of the parents. This year, the notary issued the so-called "Application" to various authorities on demand. The text seems to be normal .But here's the very name of the document! ... Is that normal, right? The main thing is that the document is drawn up on behalf of the parent not accompanying the trip (the name must match the one entered on the parent's birth certificate) and his notarized consent to take the child abroad. It is usually titled "To the Competent Authorities, to all concerned".

By the way, we crossed the border several times and did not suspect that the documents were not in order: the daughter's date of birth was 11 months later (instead of 01 it was 12 months). Fortunately for us, the border guard noticed this at the moment when my husband was nearby and everything ended well, in the sense that we left Ukraine without any problems.

Here I do not quite agree with you: in TA we were warned that without the consent of the father I should not take my child out, i.e. it turns out that either the presence of the father during the pass or his consent. But we took a chance, because. there is no connection with the father. And we have the same strict legislation.
