If you have a biometric passport, then there will be no problems crossing the border.

What about a biometric passport?
With a work visa, you can go to Italy, you just need to prove to the border guards that you are going on vacation - hotel reservation, return ticket.

And I can go visa-free, as if my visa was canceled in order to stay there longer, my work visa just ends in mid-July, I would like to stay there until August

Why cancel? Visa-free gives 90 days, this will be enough "until August".

When I am at passport control, they will let me in with a visa, since it is valid, and I have a visa for two weeks, how can I go without a visa for a couple of months?

I don’t understand how the Polish visa relates to Italy? If you are flying on a direct Ukraine-Italy flight, then you can safely use a biometric passport (which gives 90 days) + documents confirming your "tourist intention".
